Ultrasound-Guided Quadratus Lumborum Block Using Bupivacaine versus Bupivacaine-Dexamethasone for Postoperative Analgesia in Laparoscopic Cholecystectomy

Abstract

Introduction: Laparoscopic cholecystectomy (LC) is a proven, gold standard surgical procedure for 
management of gallbladder stones. Patients and Methods: After ethical committee approval and 
written informed consent was obtained from all patients, this prospective randomized double blinded 
controlled study was carried out at El-Minia University Hospital during the period from March 2018 
to December 2018. A total of 90 patients of either sex, their age ranged from 18-60 years ,American 
society of anesthesiologists class I or II were scheduled for elective laparoscopic cholecystectomy 
surgery under general anesthesia. Results: A total of 100 patients were enrolled in this study, 10 
patients refused to participate, five patients (two patients in group D and three patients in group B) 
were excluded from study due to conversion of the operation into open cholecystectoy and replaced 
by other patients, 90 patients were randomized into three groups (n=30). Discussion: Laparoscopic 
cholecystectomy is gold standard surgery for symptomatic gall stone disease which is the commonest 
disease needs surgical management. Laparoscopic cholecystectomy offers various advantages over 
open cholecystectomy like short duration of hospital stay, better recovery, lesser morbidities and less 
postoperative pain. Recommendation: Based on the current study,
Quadratus lumborum block is recommended as effective,safe and easy technique for post-operative 
analgesia in laparoscopic cholecystectomy.Further studies probably are required on a larger sample 
size to confirm our results. Future studies may be needed to assess pain beyond 24 hours
