Quality of life improvement after endoscopic management of acromegaly caused by growth hormone secretory adenomas.

Abstract:

Objective: to evaluate clinical improvement and quality of life of acromegalic patients after endoscopic endonasal trans-sphenoidal excision of GH secreting pituitary adenomas and its success rate.

Methods: This is a retrospective and prospective study on 20 patients who were admitted to Neurosurgery Department, Minia University hospital in the period from January 2018 to May 2024 with acromegaly due to pituitary adenoma. All patients underwent trans-sphenoidal excision of GH pituitary adenomas.

Results: The age of the studied patients ranged from 24 to 59 years with an average of 42 ± 8.7 with nearly equal gender presentation. There was statistically significance improvement of features, biochemical remission of GH and IGF-1 serum level, preservation of cardiac function and cardiomyopathy improvement in about 70% of included cases after trans-sphenoidal excision of GH secreting pituitary adenomas which result in decreasing of morbidity and mortality rates of acromegaly.

Conclusion: Totally endoscopic endonasal trans-sphenoidal excision of GH secreting pituitary adenomas in patients with acromegaly plays a significant role in quality of life improvement of those patients through stopping deterioration of cardiac functions, cardiomyopathy occurrence and improvement of external features by decreasing circulating GH and IGF-1 which end in decreasing of mortality rate in such patient.
