Study of serum Fetuin-A in type 2 diabetic nephropathy with and without dialysis.

Abstract

Background: Liver cells produce the glycoprotein fetuin-A, which is released into the bloodstream in large quantities. Because fetuin-A binds to the insulin receptor tyrosine kinase in muscle & fat, it causes insulin resistance in these tissues.

Aim: To assess the association among plasma fetuin A & type2 diabetes mellitus, whether or not there are microvascular complications (like diabetic nephropathy), & to comprehend how fetuin-A functions as a calcification inhibitor in the pathophysiology of diabetic nephropathy.

Patients & methods: This research had been retrospective cross-sectional research that was performed in the El Minia University Hospital over 80 cases separated into three groups: Group A: thirty studied cases have type2 diabetes mellitus with diabetic nephropathy (on hemodialysis). Group B: thirty studied cases have type2 diabetes with diabetic nephropathy (not on hemodialysis) and Group C: 20 healthy control subjects.

Results: There had been a statistically significant difference among the studied groups regarding the mean arterial pressure after operation. The sensitivity of using Fetuin-A to predict diabetes was 48.3percent, specificity was 100percent, positive predictive value had been 100percent, negative predictive value had been 39.20% & accuracy was 61.3%

Conclusion: Fetuin A levels are associated with insulin resistance, dyslipidemia, & vascular complications commonly seen in DN. Higher fetuin-A levels in dialysis studied cases reflect advanced kidney dysfunction. Fetuin-A's potential as a marker for disease severity and progression in DN is evident
