Background: Acute organophosphorus poisoning (AOPP) accounts for excessive morbidity and mortality around the world, in particular throughout the developing countries like Egypt yet no definitive predictors of outcome have been established. Aim: Evaluation of the sequential organ failure assessment (SOFA) score as a predictor of morbidity and mortality in AOPP patients. Patients and Methods: A clinical prospective study that was carried out on 80 adults of both sexes and aging ≥ 18 years, who were diagnosed with AOPP and admitted to Minia University Hospital, Poison Control Center (PCC), during the period from May 1st, 2023 to April 30, 2024. Socio-demographic, toxicological data, clinical examination, involving vital signs and the Glasgow Coma Scale (GCS), length of hospital stay and outcome of patients were collected. Peradeniya Organophosphorus Poisoning (POP) scale divided patients on admission into mild, moderate, and severe. Blood samples were collected for the routine laboratory investigations. The SOFA score was estimated upon admission (day 1) as well as after 24 hours (day 2). Results: There was a statistically significant difference between complicated and dead cases compared to non-complicated and survived cases respectively regarding the POP score on admission and SOFA score on days one and two. It was noticed that complicated and dead cases showed higher POP score on admission and SOFA score on days one and two.

Conclusions: SOFA score is highly related to the outcome of acute organophosphorus poisoning, and can be utilized to predict morbidity and mortality of patients.
