Radiological And Functional Outcomes Of Approach Choice In Surgical Management Of Transverse Acetabular Fractures

Abstract

Background: A transverse acetabular fracture merely affects the acetabulum only and not extending to the obturator ring. Both the columns of the acetabulum are affected to be considered a transverse fracture. Conventional methods of therapy, such as the combined double or extensile approach with anterior and posterior column plating are now considered aggressive treatment.



Aim and Objectives; To radiologically and functionally assess the postoperative outcomes of patients with transverse acetabular fractures fixed through kocher langenbeck approach versus anterior intrapelvic approaches.



Patients and Methods; This prospective randomized controlled study included 28 patients who came to Minya University Hospital Emergency Unit with a transverse acetabular fracture between 2021 and 2024.



Results; Age, sex, injury mechanism, comorbidities, preoperative displacement, postoperative reduction quality according to Matta criteria, and Merle d'Aubigne score were not substantially different between the groups .



Conclusion: Transverse acetabular fractures treated surgically using anterior or posterior approaches achieved comparable outcomes .
